Added Transfer Institution and Transfer Course to Evaluation Tracker E-mail Messages, added user comments, added >2 days, and fixed Send E-Mail Reminder

When creating an Evaluation Task in the Evaluation Tracker workflow, an e-mail alert can be sent each step of the way. The e-mail message simply mentioned that "A course evaluation task(s) has been assigned to your queue." It was static text that could not be updated. The "youngest" e-mail reminder is >5 days. The e-mail reminder button was disabled, unexpectedly.

How it works:

This enhancement added both the Transfer institution as well as the transfer course(s) that comprise the evaluation task. A comment box was also added so that the Evaluation Task creator can add comments to the Evaluation Tracker alert. Added a filter for evaluation tasks that are older than 2 days. The e-mail reminder "Send" button was fixed.
